Concierge Medicine Today evaluated and reviewed Top Physician Leaders in Concierge Medicine applications and answers from patients, peers, business leaders, coworkers, and/or inter- and intra-organizational colleagues and were selected by the trade publications Editor-in-Chief alone based on the many answers to the following three criteria:
The Practice of Self-Leadership. Self-leadership involves understanding oneself, managing emotions and behaviors, setting goals, making decisions, and being proactive, self-disciplined, and adaptable.
The Practice of Risk Taking. Risk-taking involves making decisions or engaging in activities with uncertain outcomes. It is essential for personal and professional growth.
The Practice of Delegation. Delegation empowers leaders to entrust tasks and responsibilities to others, harnessing their teams’ diverse skills and talents for greater productivity.

Applicants were encouraged to share their insights on many significant initiatives the physician has participated in, their past achievements, and future goals. The application also allowed space for brief examples and stories that demonstrated how these individuals exhibited healthcare leadership over the past 12 to 18 months (or sometimes more). This information was intended to provide a more comprehensive view of each candidate and illustrate their qualifications as a Top Physician Leader in Concierge Medicine.

“In order to prevent doing harm in medical practices, we need to decrease the number of patients that each doctor cares for daily to give him/her the time to adequately address the patients needs, be able to critically think about each clinical situation, and coordinate care with other practitioners if needed,” says one Concierge Medicine Physician from Connecticut. “It’s not ‘fair’ to patients to rush them in-and-out of the office, merely treating symptoms without getting to the root cause of their complaints. Building a trusting relationship with patients is at the core of what concierge doctors do on a daily basis. I’m hopeful that every young physician sees a future in which they can practice high-level medical care, achieve a work-life balance, and form a bond with their patients that will last a lifetime.”

Top Physician Leaders in Concierge Medicine nominees must either work or have worked in or for a concierge medicine (or membership-driven) practice or served in a leadership capacity for an organization serving the space within the past 12-18 months.
It should be noted, there’s no fee, no payment required, nor an award to purchase to make this list, nor is it considered an endorsement, nor is everyone who applies accepted or chosen. This is meant to be recognition of leadership within their given field, not care quality. Listing (below) is recognition of their leadership accomplishments and pursuits in its purest form only. This is also not a paid sponsorship program, nor an endorsement by Concierge Medicine Today. It’s purely and simply, recognition of leadership skills. Additionally, some Physicians have been selected by Concierge Medicine Today that were not nominated but continue to lead exceptionally well within the concierge medicine and related spaces and places.

In 2010, Dr. Gamble came to realize that our current health care system, which is based on illness and injury, fails every patient at almost every visit. She opened Greenwich Pure Medical, LLC to proactively work towards health and wellness with each patient on an individualized basis.
Realizing that neither medical school nor traditional medical training taught her anything about the role of nutrition in the prevention and progression of disease, she obtained her advanced certification in plant-based nutrition through the T. Colin Campbell foundation at Cornell University. She has been integrating the basic tenants of health through nutrition since that time.
Dr. Sarah Mildred Gamble obtained her Medical Degree in Osteopathic Medicine from the Kansas City University of Medicine and Biosciences in 2004. She completed her internship and residency training in Internal Medicine through New York Medical College at the Westchester Medical Center in Valhalla, New York. She obtained her board certification in Internal Medicine in 2009 and recertified as a diplomat in 2019. Dr. Jani exemplifies the heart and soul of concierge medicine. From the moment he boldly became the first concierge doctor in his area more than a decade ago, he has dedicated himself to making each patient feel heard, well-cared for and a very special part of his practice. It’s a great way to promote local business and also teach people about healthy eating.”
His excellence as a concierge doctor has been recognized numerous times, with awards for Delaware’s Top Integrative Medicine Physician, the Face of Integrative Medicine in Delaware, and CMT’s Leadership in Concierge Medicine. Additionally, he is an in-demand speaker at the Concierge Medicine Forum each year, generously sharing his expertise and enthusiasm with attendees at all different points in their embrace of the model.
Most meaningful to him is the powerful physician-patient connection that defines concierge medicine. Dr. Jani says: “It would not be possible to touch as many lives so personally without this model.” He talks movingly about a patient who passed away two weeks after being diagnosed with advanced liver disease. “There was no treatment that would have made a difference, but as a concierge doctor I was able to do things that could not have happened in a traditional practice. Shalini Kaneriya, MD
When Dr. Shalini Kaneriya converted to concierge medicine in 2018, her goal was to build a culture of integrity, trust and accountability in an environment centered completely around her patients. Six years in, she not only accomplished everything she set out to do, but has expanded her practice, Lifetime Internal Medicine, with the addition of two like-minded physicians who share her vision. The collective impact the team has made in caring for patients in their northern Virginia community is a remarkable tribute to Dr. Kaneriya’s dedicated leadership.
“There was never enough time before this to care for patients proactively, not reactively,” she says. “My approach is the opposite of formulaic – exploring each patient’s individual health goals, what motivates them, and then providing them with the necessary resources to lead vigorous and gratifying lives, without significant illness, disability or loss as they age.” She describes her diagnostic abilities as a gift, and delivering concierge care to her many elderly patients with chronic, complex disease as a real privilege.
According to Dr. Kaneriya, getting the best outcome for patients with an individualized treatment plan is an enormous responsibility, often entailing collaboration with specialists, exploration of more complex medications, and tackling a mountain of paperwork for prior authorizations, charting, reviewing labs, etc. It’s why she focuses on a team-based model of care at the practice, and why her search for additional partners was a process that required time, diligence and a willingness to dig in to identify the ideal fit.
She feels fortunate to have made the change when she was in her early 40s, despite the advice from many who said concierge medicine is what physicians do when they’re 55 or 60, as a road to retirement. “But that’s actually an old way of thinking. My practice filled up quickly after launch, and I knew I needed to add to my team. It's easy to find doctors whose motivation is making more money or having plenty of time to golf,” she says, “but that is not a reason to change.”
After casting a wide net by posting on social media, attending every hospital mixer she could and constantly networking with peers, she was fortunate to find two doctors who met her criteria. “Most likely to succeed are doctors who have been in practice for about 20 years, are well-known among local specialists, have an amazing reputation, but are fast approaching burnout and open to a new solution. Their reason for converting to concierge medicine is because they love taking care of patients and realize the way they’re currently practicing wasn’t helping them at all.”
Now she’s gratified to report:
“My staff is happy, well rested, well fed, and looking forward to a future with me. I'm so fortunate to be practicing with two doctors I completely respect and admire. And to be able to make this difference in my patients’ lives - be happy for them when they’re doing well and give them help and hope when they’re ill - is the best feeling in the world.”

After more than 45 years in practice, by 2021 Dr. Irene Kazmers had reached the proverbial crossroads in her career journey. One route led to closing the doors of her rural Michigan rheumatology practice, the other to making a bold leap to concierge medicine. It comes as no surprise to anyone who knows her lifetime of dedication to outstanding patient care that Dr. Kazmers chose change. By doing so, she transformed the lives of her staff, patients, family, and at long last, herself.
The struggle to maintain adequate financial margins in a time of declining reimbursements, and provide comprehensive care for more than 20 medically complex patients each day was taking a significant toll on Dr. Kazmers. Her bag of patient visit paperwork, filled to capacity nightly, became a tangible symbol of the heavy administrative burden placed on all practicing physicians in the last decade. For rheumatologists in particular, the work needed to ensure that patients receive appropriate but extremely expensive specialty medications, is voluminous. The scarcity of rheumatologists in Dr. Kazmers’ area exacerbated the issue, leaving one physician to manage care for thousands of chronically ill patients within systems that don’t allow the time to really care for them.
Dr. Kazmers says the concierge model has enabled her to do more than simply keep her practice open, but has “restored my practice, rejuvenated my staff and allowed me to care for patients exceptionally well in a time of great change and opportunity for rheumatology.”
The differences made possible with a smaller patient panel were quickly evident: available appointments, visits that start promptly, time to coordinate care with other providers, and the opportunity to incorporate integrative medicine modalities - breathing meditation, mindfulness, certified massage therapy, movement therapy, dietary approaches and nutraceutical supplements - into individual treatment plans.
Dr. Kazmers is an active and impassioned advocate for concierge medicine, describing the benefits to peers at rheumatology conferences and in professional journals. As she shared in Rheumatology Today: “The most important change has been the incredible lifting of work pressure, resulting in a much better quality of life. "Upon meeting Dr. Monaco, it becomes immediately clear why he should be recognized as the 'TOP Physician'! Dr. Monaco truly stands out. While many patients might say their doctor is the best, experiencing his care firsthand reveals just how extraordinary he really is. He is not a doctor who rushes through appointments; instead, he patiently listens to his patients’ concerns without interruption. You can see him actively engaged, thoughtfully processing information. He goes the extra mile—holding hands, offering comforting hugs, and even accompanying patients to the door, elevator, or car if needed. The visit doesn't end in the exam room; he asks questions, repeats them to ensure understanding, and takes thorough notes. I used to believe my family doctor was the best after 27 years of care; I was mistaken. Many patients don’t realize that when scheduled with a mid-level provider instead of their physician, it often means their doctor has a practice too large to manage personally. At 37, Dr. John Moultrie was too young to longingly recall the era of popular TV doctor Marcus Welby, MD but experienced enough to realize that our flawed healthcare system was not healing itself, and in fact, beset by ever increasing ills. The grind of seeing hundreds of patients each week but knowing none of them well and evenings filled with endless administrative tasks contributed to an ongoing drumbeat of professional dissatisfaction. Employed immediately after residency by a large hospital system in Atlanta, Dr. Moultrie saw his patient panel grow to unsustainable levels. “I poured my heart and soul into trying to provide concierge-type care,” he says.” But that became impossible with more than 6,000 patients, and the need to respond to all requests within 48 hours. I left every appointment wishing I had more time to delve into patients’ issues and implement changes.”
He realized that the lack of support from the healthcare system was at the root of being unable to serve such a large panel well. “While I wanted to provide the most in-depth care possible to as many people as I could, I had to come to terms with the fact that we all have limits on our bandwidth and capacity to give.” After thoroughly exploring his options, he chose to work with a consulting company whose concierge medicine model had a proven record of success, and enabled physicians to practice independently and autonomously. While he was the youngest physician the company had converted to date, his determination didn’t waver.
“I never felt I was too young to do this,” he says. “The short-term risk is well worth the long-term reward of spending most of your career practicing medicine and building relationships with patients the way you always envisioned and enjoying a personal life.”
Confirmation arrived shortly after his launch as his practice filled quickly with patients eager for holistic, preventive care. “I’m able to spend the time now with patients to dig in and discuss what’s driving their underlying stressors or unhealthy behaviors,” he says. “We have more data than ever before at our disposal, and the ability to screen for and treat conditions like heart disease, cancer, dementia, metabolic dysfunction much earlier. David S. Oyer, MD
While others may lay claim to being a prescient early adopter of concierge medicine, for Chicago’s Dr. David Oyer it is a genuine and compelling truth. In 2010 this gifted physician launched one of the first concierge endocrinology practices in the country. It was a courageous move at a time when concierge physicians numbered between 2,400 and 5,000 in total, and only 20% practiced a sub-specialty such as endocrinology. Yet, that’s where Dr. Oyer saw tremendous potential to make a difference, with the ability to provide extra time, attention and frequent monitoring for patients with a range of metabolic conditions, including thyroid disease, osteoporosis, high cholesterol, obesity, and especially, diabetes.

“You won’t fix the healthcare system by working yourself to death,” says Dr. Walton.
Dr. Walton knew she was finally ready to make the change to concierge medicine, without reservations or guilt. After considering her options - starting her own concierge practice, joining a physician with an established concierge practice, or making the change while staying in her fee-for-service physician group – she chose the latter. “I liked my partners and they wanted me to stay, so it was the right decision for me,” she says.
Working with experienced industry consultants, she crafted a concierge practice structure that allowed her to care for 500 patients instead of 3,000, and offer them same-day, longer appointments and 24/7 direct communications. The model proved immensely appealing to her current patients, who signed up quickly to become members of her practice.
