What happens next

From application to your table

Apply

Choose your options, accept the agreements, and sign as an authorized representative of your organization.

We review

We review your application for fit and availability, usually within 5–10 business days.

Confirm & pay

If approved, you receive written confirmation and an invoice. Benefits activate only after payment.

Prepare

Send staff names and your Certificate of Insurance by October 1. Check in on arrival, then set up.

  • Primary contact and authorized signatory
  • Organization name, business address, phone, and website
  • Your company logo (high resolution)
  • Names of team members who need badges (2 badges per table; extra badges available)
  • Invoice email, if different

Good to know

  • Sponsorship fees are non-refundable. Taxes, catering, and processing fees may apply.
  • No on-site sales, order-taking, or payment collection
  • CMF assigns table placement and may move tables; no booth numbers
  • Attendee list after check-in; email only, two emails, by November 16 following the event
  • Insurance: CGL incl. products-completed operations, $1M / $2M, naming Concierge Medicine Today, LLC
  • All sponsor staff must be 21 or older
