Readiness and Model Check
Is this right for me? Look honestly at whether you are ready for a membership-based practice, and which model fits your life and your numbers.
“You didn’t become a doctor to just survive it. You became one because of a calling. CMF exists to help you build a practice, and a life, you’re in it for. For the patients. For the joy.”Michael Tetreault, Editor-in-Chief, Concierge Medicine Today. Welcome to the 2026 Concierge Medicine Forum.
You are not behind. You are deciding something that deserves time, honest questions, and good advisors, not hurry.
What you will walk away with
- Your readiness phase, Learn, Plan, or Launch, across mindset, business, and marketing
- Directional ranges for panel size, fees, and overhead across common practice models
- The two regulatory lanes that shape which models are open to you
- A clear next step for the phase you are in
Before you start
This tool is for reflection and education. It does not tell you whether you should make this change, and it is not legal, financial, tax, or medical advice. Figures are directional ranges from published benchmarks, not projections for your practice. Nothing you enter leaves your browser. Read the full disclaimers, methodology, and sources.

Part 2: Your model
These inputs shape the model ranges in your results.
Methodology, limitations, and sources
How it works
- Readiness. Each “Yes” counts one point. “Not yet” and “Unsure” count zero. 0 to 5 points is the Learn phase, 6 to 10 is Plan, and 11 to 20 is Launch. The statements and thresholds are CMT editorial judgment, not a validated instrument.
- Model ranges. Panel, fee, overhead, and recurring-revenue ranges come from the published benchmarks below. Fees are adjusted directionally for market (major metro 1.15x, rural 0.82x) and role. Illustrative revenue multiplies the midpoint fee by your panel, held within each model’s typical range.
- Closest fit is based on panel size alone and does not account for your goals, patients, or market.
- Your insurance preference marks which models are generally compatible with that lane. It is a summary, not a legal determination.
Limitations
- Ranges are directional, cross-market figures. Actual results vary widely by specialty, geography, payer mix, demand, and execution.
- Scope of practice for NPs and PAs, and the role of RNs, vary by state and are changing. Verify with your state board.
- Some benchmark sources below are industry or first-party publications rather than peer-reviewed research.

- Macbach. 2026 Concierge Medicine Benchmark Report: retainer bands, overhead, and recurring-revenue share by tier. Proprietary benchmark data from a healthcare marketing agency’s own client engagements across a multi-state roster, not an independent industry-wide survey. Note: Macbach publishes an open-ended “$15,000+ per year” floor for ultra-premium concierge and treats performance medicine ($48,000+ per year) as a separate category. The $45,000 ceiling shown for bespoke concierge is CMT’s own editorial boundary, set below that category, not a Macbach figure.
